Community Rehabilitation
Community Rehabilitation (CR) provides multidisciplinary rehabilitative therapy to patients aged 16 years and above living in the designated EMHS / RPBG - Bentley Health Service CR postcode catchment area.
The multidisciplinary team aims to provide a coordinated patient-centred service to patients with an impairment, activity limitation or restriction due to a health condition or following an injury.
The team includes:
- clinical psychology
- dietetics
- medical
- nursing
- occupational therapy
- physiotherapy
- speech pathology
- social work.
CR services are assessment based, goal oriented and time-limited. The services are usually delivered in the outpatient setting in D Block, however they may also be delivered in the patients’ home (for example a Falls Specialist, or OT home visit to assess access/equipment), or other community location appropriate to the patients’ goals.
