Complex Needs Coordination Team

The Complex Needs Coordination Team (CoNeCT) operates across the East Metropolitan Health Service area, with small teams based at Royal Perth and Armadale Hospitals.

Patients who are identified by hospital staff as being at risk of further hospitalisation are supported by CoNeCT Care Coordinators to manage their healthcare at home through chronic condition self-management support, linkage to appropriate services and coordination of complex care needs.

Information for GPs

Referral to CoNeCT by GPs will be considered if the patient has a recent history of frequent Emergency Department presentations which could have been avoided through care coordination intervention in the community.
